I have just one comment regarding the degree of difficulty. The reason to include such a category has nothing to do with confidence in diagnosis, etc., and everything to do with a meaningful retrieval after the database gets very large. When you have hundreds of cases of some unusual lung disease, you want some ways to pick out the cases of interest for a particular conference or retrospective research study.
