Primary CNS lymphoma in a 50-year-old man with positive test results for the human immunodeficiency virus and progressive somnolence. Panel A Axial T2-weighted MR image shows a hypointense mass (arrow) in the lateral aspect of the corpus callosum, just superior to the roof of the lateral ventricle. There is surrounding hyperintensity, indicative of edema. Panel B Coronal contrast-enhanced T1-weighted MR image shows the ring-enhancing mass (arrow) in the lateral aspect of the corpus callosum, abutting the roof of the right lateral ventricle. Panel C Photograph of the coronally sectioned autopsy specimen demonstrates the callosal mass (arrow) with reddish-brown discoloration secondary to hemorrhage. The mass, which was partially necrotic at histologic examination, abuts the ventricle and extends superiorly into the white matter of the right cingulate gyrus. Histologic examination revealed primary CNS lymphoma.
