Panel A, Panel B Unenhanced (A) and contrast-enhanced (B) axial CT scans show an ill-defined area of high attenuation (arrows) in the left frontal region that mildly enhances after administration of contrast material. Panel C Sagittal T1-weighted MR image shows an extraaxial mass (arrows) that is isointense to mildly hypointense relative to brain parenchyma. Panel D On a sagittal contrast-enhanced T1-weighted MR image, the mass (arrows) enhances intensely. A dural tail sign (arrowhead) is seen anteriorly. Panel E Coronal contrast-enhanced T1-weighted MR image reveals that the mass envelopes the superior sagittal sinus (open arrow) and crosses the midline (solid arrow). Panel F Lateral external carotid arteriogram (arterial phase) shows an early blush (arrow) in the region of the mass, which was primarily supplied by the left middle meningeal artery. Preoperative embolization was subsequently performed. Panel G Intraoperative photograph shows the cut edge of the dura mater (black arrows) and a yellowish mass (arrowheads) that was firmly adherent to the dura mater, which is retracted (white arrows). The superior sagittal sinus was not involved by tumor. Although the imaging and operative findings are characteristic of an en plaque meningioma, histologic analysis revealed a B-cell lymphoma.
