Primary CNS lymphoma in a 70-year-old man with intermittent syncopal spells who became unresponsive after a fall. Panel A Contrast-enhanced axial CT scan shows intensely enhancing masses of the midbrain. The larger mass (arrow) is comma-shaped and occupies most of the right cerebral peduncle. The smaller mass (arrowhead) lies in the left cerebral peduncle. Panel B Axial photograph of the autopsy specimen shows the comma-shaped mass (m) of the right cerebral peduncle and the smaller well-defined mass (arrowhead) of the left cerebral peduncle. The right-sided mass also extended into the right thalamus (not shown). At histologic analysis, a large cell lymphoma was found.
