Primary CNS lymphoma in a 20-year-old man with nausea, weakness, diplopia, unsteady gait, and an 80-pound weight loss. Multiple cranial neuropathies were evident and involved cranial nerves V3 (right), VI (bilateral), and VII (right). Panel A Unenhanced sagittal T1-weighted MR image shows an ill-defined area of isointensity (arrows) surrounding the fourth ventricle and a mildly hypointense nodule (arrowhead) of the posterior aspect of the medulla. Panel B Contrast-enhanced sagittal T1-weighted MR image shows intense enhancement of these same areas. Pathologic findings revealed malignant lymphoma.
