Figure 13: Primary CNS lymphoma in a 37-year-old man with AIDS. Panel A Unenhanced axial CT scan shows a large mass (arrow) with a central low-attenuation area and a peripheral high-attenuation rim and with extensive surrounding edema and mild mass effect on adjacent sulci and the left frontal horn. The scan was obtained after 2 weeks of antitoxoplasmosis therapy and showed increased size of the lesion compared with prior scans (not shown). Panel B Contrast-enhanced axial CT scan, obtained 2 days later after the patient became moribund, shows intense ring enhancement (arrow) along the lateral and posterior aspects of the mass, with increasing moderate to severe mass effect, midline shift, cisternal effacement, and uncal herniation. Panel C Photograph of coronal slices of the autopsy specimen shows the large necrotic mass (M) of the left basal ganglia with surrounding brownish discoloration and left uncal herniation.
