Figure 10: Primary CNS lymphoma in a 57-year-old woman who experienced progressive lethargy and weakness over a 6-month period. Panel A, Panel B Axial proton-density-weighted (A) and contrast-enhanced T1-weighted (B) MR images show diffuse enlargement and hyperintensity (arrow in A) of the right cerebral peduncle with intense focal nodular enhancement (arrowhead in B). Panel C Photograph of the axially sliced autopsy specimen shows corresponding focal necrosis (arrow) of the right cerebral peduncle. Panel D Axial T2-weighted image shows ill-defined hyperintensity of the right basal ganglia (b), which is also enlarged. Panel E Photograph of the axially sliced autopsy specimen shows diffuse involvement of the right putamen (b), globus pallidus, and caudate head (c), which correlates with the MR imaging findings. Panel F, Panel G Axial T2-weighted (F) and contrast-enhanced T1-weighted (G) MR images show heterogeneous signal intensity of the right periventricular area (d) with surrounding hyperintensity consistent with white matter edema. Intense focal enhancement (arrow in G) is seen with administration of contrast material. Panel H Photograph of the axially sliced autopsy specimen shows diffuse involvement of the right caudate nucleus (n), which is enlarged and has a mottled appearance indicative of necrosis. Histologic examination revealed primary CNS lymphoma.
