Nonfunctioning islet cell tumor in a 60-year-old man in whom a palpable mass was found at routine physical examination. Panel A Contrast-enhanced CT scan shows a heterogeneous, lobulated, 13-cm mass in the pancreatic head. The mass contains a punctate central calcification (arrowhead) and areas of low attenuation (arrows). Panel B Axial contrast-enhanced T1-weighted (800/20) image demonstrates an area of heterogeneous enhancement with a central area of high signal intensity (solid arrow) that most likely represents cystic degeneration. The posterior high-signal-intensity region (open arrow) contained hemorrhagic debris. Panel C Axial T2-weighted (1,846/80) MR image shows homogeneous high signal intensity of both hemorrhagic (open arrow) and nonhemorrhagic (solid arrow) cystic degeneration. Panel D Photograph of the gross specimen shows the dominant cystic cavity (arrow) and other smaller areas of cystic degeneration.
