Figure 6: Insulinoma in a 65-year-old woman with multiple medical problems who was admitted for symptomatic hypoglycemia. Panel A Axial contrast-enhanced CT scan demonstrates a hypervascular mass within the pancreatic head (arrows) that measured approximately 2.5 cm in diameter and produced mass effect on the superior mesenteric vein. Panel B Corresponding axial inversion-recovery (4,000/32, 90° flip angle) MR image demonstrates the mass, which has homogeneous high signal intensity (similar to that of cerebrospinal fluid). This is a helpful characterizing feature in cases of islet cell tumors. Panel C Superior mesenteric arteriogram demonstrates the hypervascular nature of the tumor and its arterial supply. Panel D Intraoperative US scan demonstrates the sharply marginated mass, which is predominantly hypoechoic. Note the small anechoic areas of cystic degeneration that were not seen on the CT or MR images.
