Nonhyperfunctioning islet cell tumor in a 45-year-old woman with an 8-month history of abdominal pain. Panel A Axial sonogram demonstrates a mass (arrows) of heterogeneous echotexture with a large, central anechoic area. Panel B Axial contrast material-enhanced CT scan shows the 10-cm peripherally enhancing mass in the pancreatic tail. The central area of low attenuation represents cystic necrosis. Panel C Axial T2-weighted (repetition time msec/echo time msec = 5,705/90) MR image shows a high-signal-intensity central cyst with an irregular peripheral rind of tumor. Panel D Axial gadolinium-enhanced T1-weighted (685/15) image reveals the large, peripherally enhancing mass in the pancreatic tail. The central region of low signal intensity corresponds to cystic degeneration. Panel E Photograph of the bivalved gross specimen reveals the large, single central cyst, which corresponds to the findings on US, CT, and MR images.
