(A and B) Anteroposterior Panel A and lateral Panel B lumbar radiographs show a destructive lesion (arrows) at L-3 involving the vertebral body and left pedicle with lateral compression. Panel C CT scan reveals a predominantly lytic lesion (arrows), although there is a sclerotic component posteriorly (arrowheads). (D and E) Sagittal T1-weighted Panel D and T2-weighted Panel E MR images reveal extensive marrow replacement (large white arrows) and an anterior epidural mass (small and black arrows) with intermediate signal intensity on both images. Posterior soft-tissue changes are from previous surgery. Panel F Photomicrograph (original magnification, X125; hematoxylin-eosin stain) shows sheets of monotonous, small, blue, round cells.
