Osteochondroma of the sacrum with malignant transformation to a chondrosarcoma in a 35-year-old woman with low back pain and a palpable mass. Panel A Radiograph of the pelvis reveals vague sclerosis (solid arrows) over the left sacrum and widening of the sacroiliac joint (open arrow). Panel B Coronal reconstructed CT scan shows the cortex and marrow canal of the osteochondroma (arrows) and continuity with the sacrum (arrowheads). Panel C Axial CT scan shows the thick cartilage cap (arrows) and sacroiliac joint invasion, which represents malignant transformation. Panel D Photograph of a coronally sectioned gross specimen reveals a large cartilaginous mass (*) with involvement of the sacroiliac joint (arrows). At histologic analysis, a grade 1 chondrosarcoma arising in an osteochondroma was diagnosed.
