Figure 8: Ileoileocolic intussusception in a 7-year-old-girl with CF and a 1-week history of constipation, emesis, and periumbilical pain. Panel A Supine, frontal radiograph of the abdomen displays a pattern of distal intestinal obstruction, a paucity of right flank gas, and a soft-tissue mass partly outlined by gas in the right subhepatic region (arrowheads). (B and C) Longitudinal Panel B and transverse Panel C sonograms of the right upper quadrant soft-tissue mass demonstrate "pseudokidney" and "target" configurations, respectively, characteristic of intussusception. Panel D Overhead radiograph obtained after administration of a Gastrografin enema shows a lobulated filling defect in the midportion of the ascending colon (arrowheads), associated with a "coiled spring" appearance of approximated mucosal folds. The intussusception could not be completely reduced by the contrast material enema procedure, and a distal ileal resection was performed. Panel E Cross section of the resected ileoileal intussusception, corresponding to the transverse sonogram , shows the whorled pattern of bowel within bowel. The inner layer (arrowheads) represents the intussusceptum. Necrosis is present in the outer layer (intussuscipiens) (arrows). Scale is in centimeters.
