Figure 16: Well-differentiated hepatocellular carcinoma in a 33-year-old man who presented with right upper abdominal pain. Panel A Contrast-enhanced CT scan shows a large mass in the left lobe of the liver. Heterogeneous peripheral enhancement and a central low-attenuation area (arrow) representing a scar are noted. Panel B CT scan obtained at a lower level shows two low-attenuation areas (arrows) at the margin of the mass, suggestive of necrosis. Panel C Gadolinium-enhanced MR image (150/5, 80¡ flip angle, multiplanar gradient-refocused technique) shows the mass, which has central low-signal-intensity areas of necrosis and scarring with peripheral enhancement. Panel D On the T2-weighted MR image (3,000/60), the mass has high signal intensity, but the central scar remains low signal intensity (arrow). Panel E Gross specimen of the resected left hepatic lobe shows a large lobulated mass extending and bulging the surface of the liver. A white fibrous band (arrow) corresponding to the central scar is noted.
