Fibrolamellar carcinoma in a 15-year-old boy who presented with abdominal pain. Panel A Contrast-enhanced CT scan shows a large heterogeneous mass in the right lobe of the liver. Central calcification is noted (arrow), along with a poorly defined central scar. Panel B T1-weighted MR image (300/12) shows that the mass has slightly lower signal intensity than that of the liver. The low-signal-intensity central scar (arrow) is still poorly defined. Panel C On the T2-weighted MR image (2,000/80), the mass has inhomogeneously high signal intensity and the central scar remains low signal intensity. Panel D Gross specimen demonstrates stellate radiating white bands of lamellar fibrosis (seen as low signal intensity in c) and areas of green bile staining within the pale yellow tumor, features typical for fibrolamellar carcinoma.
