Figure 10: FNH in a 31-year-old woman, with a history of adenomyosis treated with oral contraceptive pills, who presented with diffuse abdominal pain. Panel A T1-weighted MR image (141/2.2, 60¡ flip angle, spoiled GRASS [gradient recalled acquisition in the steady state] technique) reveals a large mass in the medial segment of the left lobe of the liver. The mass is nearly isointense relative to the normal hepatic parenchyma. A central low-signal-intensity stellate scar (arrow) is noted. Panel B T1-weighted image (141/2.2, 60¡ flip angle, spoiled GRASS technique) obtained during gadolinium injection shows a similar degree of enhancement relative to the normal liver. The central scar also enhances (arrow). Panel C Delayed MR image (33/5, 45¡ flip angle, spoiled GRASS technique), obtained 20 minutes after gadolinium injection, demonstrates more prominent enhancement of the scar (arrow) due to retention of contrast material within the myxomatous portions of the scar. Panel D T2-weighted MR image (4,200/102) shows the isointensity of the mass relative to the liver. The central scar has a very high signal intensity (arrow). Panel E Bivalved specimen from the resected liver shows a lobulated mass with central scars (arrows) radiating outward in a stellate pattern.
