Complete hydatidiform mole with recurrence after initial treatment in a 31-year-old woman with uterine size greater than expected for dates. Panel A Initial US scan obtained 18 weeks after last menstrual period shows an enlarged uterus (arrows) with no identifiable fetus and ill-defined central hypoechogenicity. A diagnosis of hydatidiform mole was made, and the patient was treated with dilation and curettage. Ten weeks later, the patient was asymptomatic, but her beta-hCG levels were elevated. Panel B Sonogram shows a hyperechoic central mass with numerous cystic spaces. Panel C Computed tomographic (CT) scan shows the low-attenuation central mass with intact myometrium. Panel D Coronal T1-weighted magnetic resonance (MR) image (700/25 [repetition time msec/echo time msec]) shows the mass has slightly increased signal intensity without evidence for invasion. Panel E Hysterectomy specimen of the bivalved uterus shows the well-circumscribed central mass with numerous vesicles. No invasion was seen at histologic evaluation.
