Figure 8: Splenic pneumocystosis in a 40-year-old man with AIDS who had fever and abdominal pain. Aerosolized pentamidine was used for prophylaxis of pulmonary infection. Panel A Contrast-enhanced CT scan of the upper abdomen shows multiple low-attenuation lesions of varying size involving the entire spleen. Panel B Gross specimen of a cut surface of the spleen demonstrates multiple white-yellow nodules 0.1 2.5 cm in diameter. Panel C Low-power photomicrograph (original magnification, X13.2; H-E stain) of the splenectomy specimen shows a nodule (arrow) composed of a foamy eosinophilic material that is suggestive of pneumocystosis. Panel D High-power photomicrograph (original magnification, X250; H-E stain with light methenamine silver) shows the cyst form of P carinii.
