Disseminated histoplasmosis and perisplenic abscess in a 37-year-old man with AIDS who presented with fever and diarrhea. Panel A US scan of the upper abdomen shows splenic parenchyma (S) and what appears to be a large, perisplenic fluid collection with multiple septations. Panel B Non-contrast-enhanced CT scan of the upper abdomen shows an area of perisplenic low attenuation that displaces the enlarged spleen (S) medially. Panel C Gross specimen of a cut surface of the spleen demonstrates preserved splenic tissue (S) centrally surrounded by a 3-cm-thick, pale, pink-tan rim consistent with subcapsular coagulative necrosis. Panel D Photomicrograph (original magnification, X250; Grocott-Gomori methenamine silver nitrate stain) of the splenectomy specimen shows small budding yeast cells characteristic of H capsulatum infection.
