AIDS cholangitis secondary to CMV in a 35-year-old man who presented with nausea, bloating, and diarrhea. Panel A Ultrasonographic (US) scan of the upper abdomen shows dilatation of the common hepatic bile duct (arrowheads) and minimal dilatation of the intrahepatic ducts. Panel B Endoscopic retrograde cholangiopancreatogram shows mild dilatation of the common bile duct and irregularity of its mucosa. The distal end of the common bile duct appears to be within normal limits (arrow), although papillary stenosis was noted by the endoscopist. The intrahepatic bile ducts are slightly dilated and irregular. Sphincterotomy was performed, and biopsy of the duodenal ampulla revealed CMV inclusion bodies within mesenchymal and epithelial cells.
