CMV ileocolitis in a 31-year-old man with AIDS who presented with crampy abdominal pain and diarrhea. Panel A Radiograph from a single-contrast barium enema study shows a large mass (arrows) within the cecum with a "coil spring" appearance compatible with ileocolic intussusception. Panel B Gross specimen of the reduced ileum shows no leading edge other than edematous and somewhat hemorrhagic mucosa. Panel C Photomicrograph (original magnification, X3.3; H-E stain) of the resected ileum shows marked submucosal edema and mucosal hemorrhage. Focal erosion is noted (arrow).
