CMV ileitis in a 36-year-old woman with AIDS who presented with diarrhea and weight loss. Panel A Spot view from a small bowel follow-through examination shows narrowing and mucosal irregularity of the terminal ileum that extend to the ileocecal valve. Panel B Contrast material enhanced computed tomographic (CT) scan of the abdomen shows marked thickening of the ileal wall (arrow). Linear densities shown within the mesenteric fat are suggestive of inflammation. Panel C Gross specimen shows marked thickening of the ileal wall and edematous, hyperemic mucosa. There are multiple ulcers (arrows) throughout the thickened mucosa, some of which have penetrated deeply, almost up to the serosa. Note that the mucosa adjacent to the inflammatory mucosa is normal. Panel D Photomicrograph (original magnification, X100; hematoxylin-eosin [H-E] stain) of the resected ileum shows submucosal vessels with CMV inclusions of endothelial cells (arrows).
