Panel B Radiograph obtained 14 months later shows extensive destruction of almost the entire third metatarsal, the base of the proximal phalanx of the third toe, and the distal portion of the fourth metatarsal (arrows). Panel C Photograph of the gross specimen shows replacement of the third metatarsal and the distal portion of the fourth metatarsal (arrows) by fibrovascular tissue. The distal fourth metatarsal has a pencil-like deformity distally (red pointer). Panel D Photomicrograph (original magnification, approximately X150; hematoxylin-eosin stain) reveals vascular proliferation with variably sized vessels, including thick-walled arteries and veins (arrows), capillaries (arrowheads), and larger cavernous spaces (*).
