Figure 11: Cystic nephroma in a 22-month-old boy, in whom a right flank mass was detected during a routine physical examination. Panel A Sagittal sonogram of the right kidney demonstrates a multilocular cystic mass with smooth, thin septa. Panel B Contrast-enhanced CT scan reveals a thin rim of compressed renal parenchyma about the medial aspect of the 10-cm-diameter mass (arrows), confirming its renal origin. Arrowheads delineate the well-defined tumor capsule. Panel C Posterior Tc-99m DTPA renal scan obtained 12 minutes after injection shows a large defect in the right renal fossa containing curvilinear bands of faint radiotracer accumulation (arrowheads), probably representing summation of activity within the soft-tissue septa. The remaining poorly functioning renal parenchyma is displaced inferiorly (arrow). Panel D Bivalved gross specimen obtained at heminephrectomy reveals multiple thin-walled cysts ranging from 1 mm to 3 cm in diameter. A rim of preserved upper pole renal parenchyma can be seen (arrow).
