CPDN in a 6-month-old boy with an incidental abdominal mass. Panel A Sagittal sonogram of the left kidney demonstrates a multiloculated mass with thin septa. A beak of compressed upper pole renal parenchyma is seen adjacent to the mass (arrows). Panel B Nonenhanced CT scan shows soft-tissue septa dividing the 11.5-cm-diameter mass into multiple fluid-filled loculi. Panel C On the contrast material enhanced CT scan, the septa enhance, but contrast material does not accumulate within the loculi. Mild hydronephrosis is evident from mass effect (arrow). Panel D Photograph of the gross specimen shows the fibrous capsule of a well-circumscribed mass. Panel E When bivalved, the interior of the gross specimen reveals multiple loculi separated by fine translucent septa. Note the absence of nodular solid elements.
